Must be completed for consideration by SGA for approval.
Assumption of Risk & Student Code of Conduct Form
Must be completed each academic year by participating students prior to a planned off-campus activity.
Must be completed and attached with the Request for Funds
Southwest Scheduled Club Activity Form
Must be completed prior to any planned activity on or off campus.
Must be completed prior to depositing money into the club account.
Must be completed in the presence of an SWCC Business Office employee.
Food for Thought Donation Verification Form
Must be completed prior to requesting club funding.
