Occupational Therapy Assistant Program Application

Applications to the OTA program are being accepted through February 15, 2020.

OTA Application

  • Enter the entry year you are applying for ie., 2020, 2021 etc. Southwest OTA accepts students beginning each fall term.
  • OTA classes may be held at the main campus, Booth Center and Lebanon Center. This selection is for student preference for class/lab meetings when offered at more than one location.
  • Please provide your mailing address here, and verify that your mailing address is correct in the Student Information System. Complete a "Change of Address Form" in the Admissions Office if the SIS address is incorrect.
  • 7 digit EmplID
  • Select the county or jurisdiction in which you have lived during the last consecutive 12 months?
  • Please list all colleges previously attended, last year attended, and academic standing. (Good standing, probation, suspended or dismissed)
  • I have previously submitted official high school/home school/GED transcripts to SWCC Admissions. (Official transcripts are required by the February 15th deadline.)
  • I have previously submitted official college transcripts to SWCC Admissions. (Official transcripts are required by the February 15th deadline.)
  • Pre-requisites must be completed by the February 15th application deadline. Transcripts verifying grades/GPA must be submitted to Admissions.
  • I have completed high school level chemistry (or CHM 05 or CHM 111 or BIO 141/142) with lab with an earned grade of "C" or better.
  • I have completed high school level biology (or BIO 20 or BIO 101 or BIO 141/142) with lab with an earned grade of "C" or better.
  • I have demonstrated competency in Math (proficiency in Math Modules MTE 1-6 or completion of MTH 151 or higher) via VPT scores or means and measures identified in Multiple Measures Placement Policy.
  • I have demonstrated competency in English (place into ENG 111 or completion of ENG 111) via VPT scores or means and measures identified in Multiple Measures Placement Policy.
  • I have an earned cumulative GPA of 2.5 or higher. (Based on last school attended whether HS or college, with at least 12 credits completed.)
  • I request that my name be placed in consideration for admission to the Occupational Therapy Assistant program for the academic year.
    • I understand that I must submit my completed OTA Job Shadowing Form and my TEAS scores, by March of the application year, to the Program Director.
    • I understand that I must submit an application/reapply for each year I wish to be considered.
    • I understand that I must complete (with a grade of “C” or better) all prerequisites and I must meet all admission requirements in order to be eligible for consideration.
    • I have read and understand the residency requirements as well as the Occupational Therapy Assistant Application Process information.
    • I understand that first priority for acceptance will be given to applicants who reside within the service region of SWCC. Secondary priority will be given to Virginia residents outside of those counties, and then out-of-state applicants
    • I also understand that if I do not have all required documents submitted with this application and have all requirements met by the deadlines specified, processing of my OTA application will be suspended.

SWCC provides state of the art facilities to enrich student learning. The Virtual Cadaver Lab allows students to explore the anatomy of a human body through simulation. The Clinical Simulation Lab located in Russell Hall, provides VATNP course simulations for nursing students using high-fidelity simulation manikins.
